Create a Questionnaire

Questionnaires allow you to create custom online forms for your members, such as waivers, health forms, PAR-Qs, and more.


Navigating to Questionnaires

  1. Click Setup in the left-hand navigation sidebar.
  2. Hover over Personalize to expand its submenu.
  3. Click Questionnaires.

This opens the Questionnaires list, which displays all existing questionnaires with the following columns:

IDA unique identifier for the questionnaire.
NameThe name of the questionnaire.
MandatoryWhether the questionnaire is mandatory for members to complete.
Self RegistrationWhether the questionnaire is presented to members who self-register.
StatusWhether the questionnaire is currently Enabled or Disabled.
ActionsLinks to Enable/Disable, Edit, or Delete the questionnaire.

Creating a New Questionnaire

Click Add New in the top right of the Questionnaires list to open the Add Questionnaire screen.

Settings

Configure the following options before building the questionnaire:

Name (required)The name of the questionnaire.
MandatoryIf set to Yes, all members will be required to complete this questionnaire.
Tagged to Self RegistrationIf set to Yes, members who self-register will be prompted to complete this questionnaire.
Digital Acceptance RequiredIf set to Yes, members must digitally accept the questionnaire before submitting.

Building the Questionnaire

Use the three buttons below the settings to add fields to the questionnaire:

  • Add Text Field Only – Adds a block of text with no response field. Use this to introduce the questionnaire or provide instructions to the member. It is recommended to start every questionnaire with a Text Field.
  • Question with Text Answer – Adds a question with a free-text response field. A mandatory setting can be configured for each question individually.
  • Question with YES | NO – Adds a question with a Yes/No response. A mandatory setting can also be configured per question.

Add as many fields as needed. Each field appears as a row in the builder with a text entry area and the following controls on the right:

  • Mandatory dropdown (Questions only) — Set to Yes or No to control whether the question is required. Text Fields do not have a mandatory setting.
  • ▼ / ▲ arrows — Move the field down or up to reorder it within the questionnaire.
  • Remove – Deletes the field from the questionnaire.

Once complete, click Save and Exit to save the questionnaire, or Back to return to the Questionnaires list without saving.
